Covid vaccines for 12 to 15 year old pupils

Covid vaccines for 12 to 15 year old pupils will be offered at SMC by the City Health Care Partnership on the following dates:
Thursday 21st, Friday 22nd October, Thursday 4th, Friday 5th & Monday 8th November 2021 for those who choose to take part.
Please see the Public Health England information documents and copy of the consent form for parents/ carers below.
Paper consent forms will be distributed via the school this week and must be returned by Thursday 7th October 2021.
The IntraHealth Immunisation Team are organising the vaccinations which are being delivered by nurses from the Covid team at City Health Care Partnership. Their contact number for any queries is 01482 344075. You can also contact them on Tel 0333 358 3397 (option2, option2).
Parents/carers are encouraged to return the form even if they do not want the vaccination, there is a yes and no section on the form.
Only children with yes consent forms completed by parents will be vaccinated on the day.
The team will consist of nurses, a health care assistant, administrators and someone to monitor the children post-vaccination.
